The “Burg Aktiv Team” of the NEUE BURG is represented in the districts of NIENBURG/WESER, ROTENBURG/WÜMME, VERDEN, the HEIDEKREIS, in the northern district of Diepholz as well as in the western region of HANNOVER.

In close cooperation with doctors, authorities and other institutions, our competent and experienced nursing staff provide a wide range of care and assistance.

The aim of psychiatric home nursing is to avoid hospital stays through individual consultations in the home environment.

With our outpatient care services, we actively accompany and support the transition from an institution to everyday living.

The focus is on jointly developing solution strategies to cope with psychological problems and fears in crisis situations in order to be able to meet the demands of everyday life again.

Our nursing staff will support you in establishing a daily structure and in organizing work, employment and leisure activities.

Psychiatric home nursing promotes understanding of one's own illness and seeks, together with those affected, ways to deal with difficulties.

Further support services can also be coordinated with the specialist. With the involvement of family members or the patient's social circle, the NEUE BURG Psychiatric Home Care offers a redesign of interpersonal relationships.

Regular internal and external training of our employees, as well as supervision, are an integral part of our company.

We support people with mental health problems in their familiar environment, in everyday situations but also in times of crisis.

With our outpatient care services, we accompany and support the transition from an institutional setting to everyday life. This can be a difficult moment for many people.

Relatives and other important people for the client can be included if desired.

Psychiatric home nursing promotes understanding and awareness of one's own illness and seeks, together with those affected, ways to deal with difficulties.

As part of our work, we strive to create the most independent living spaces possible, striving to achieve the highest possible quality of life.


The frequency of visits to the patient per week depends on the needs of the mentally ill person, but a maximum of 14 sessions.

  • What are the main tasks of the pHKP?

    Building a trusting and sustainable care relationship Since insured persons with schizophrenia often cannot trust their perceptions and, for example, perceive other people as threatening, they find it very difficult to accept help and to allow contact. Securing/supporting medication treatment Without help, insured persons usually withdraw socially, stop taking medication and avoid visits to the doctor. In addition, the doctor relies on expert external observation of the effects and possible side effects of the medication. Psychoeducational work with the insured persons in which the insured person - precisely because of their self-alienation - learns to classify symptoms of illness and crisis, accept help, avoid triggering stressful situations, etc. Support in dealing with the illness appropriately This applies to both the insured person and their relatives. Due to the severity of the illness and the associated changes in behavior and disruptions in the ability to form relationships, relatives must also be counseled. Otherwise, tensions can arise, which in turn can exacerbate the crisis. During an acute phase of the illness, insured persons sometimes need to be accompanied in everyday activities that are important for treatment or for their survival. For example, while shopping, it is easier to observe whether paranoid fears, such as those of a particular building, persist than if the situation is merely discussed with the insured person retrospectively.
